Home Primary Care

We provide regularly scheduled primary care to residents living in independent retirement communities, assisted living, alzheimers, memory care and adult family homes.

Our health care professionals are dedicated to making health care more accessible to those unable to leave their home or community by providing quality health care in the patient’s home setting.

Our doctors and nurse practitioners are qualified to write prescriptions and to treat and manage illnesses.

We can assist you with injections, IV antibiotics, protimes, other lab work and diagnostic testing. Arrangements can be made for most lab draws and diagnostic testing on site. When needing a referral to a subspecialty, your primary care provider will work with the specialist to ensure the best care possible.
